Could provide up to 15 hours of professional mediation support to help a young person and their family resolve conflict, rebuild relationships and prevent a crisis that could lead to homelessness.

Could provide over 43 hours of specialised one-to-one support from a dedicated Aftercare Worker, helping a young person leaving care build the skills and confidence needed for independent living.
